Data logging phone call types not related to accounts, contacts etc

I have a client who needs to log incoming phone call type, i.e sales call, price query etc. They are not related to any particular account. I can create a data grid then right click to add a type but this is too many clicks. I need the add screen to be presented first, then once the user has selected (RadioGroup) and closed the box, then screen closes.
Any suggestions gratefully received.

You can modify the Activity screen by adding a Tab to it.
Put your Radio Button Group on it, make the tab visible only if Activity type = phone call (262146).
On the form's OnChange event you can make your tab active and viewable, hide the other tabs....when they click on a radio group item make the other tabs visible....
or pop a managed form by a button placed on the Activity form
