I want to add Quantity field from opportunity table and Notes field from Activity..How to get these tables in e-mail template.
If anyone knows pls help me ?

I believe you need to edit the "SLX Mail Merge OnCustomFieldName" script and use custom merge fields. There's examples in the script.
