lookup name changed

Yesterday i added a Code Snippet Action Item to a lookup Opportunity_JobContacts.lkuCustInsAccount_OnChange.
And it was woking fine. Today when i run the lookup I get the following error:
Could not find the dynamic method Opportunity_JobContacts.lkuCustInsAccount_OnChange. Check you method configuration to ensure that this method exists.
Now the name of the Code Snippet Action Item is 64f8520e-5948-4d86-9871-433a5281b9df.lkuCustInsAccount_OnChange.
I'm not sure how this i happening or what i'm doing wrong.
Any help is much appreciated.

Did you ever find a solution for this? I get the same error and can't figure out what is it.
