LeadCapture Error 7.5.4

fiogf49gjkf0d Hello everyone. I'm having an issue with LeadCapture not working correctly. I've tried several rebuilds and redeploy with no change.
After enabling logging, I'm getting the follow error output. Any insights would be appreciated!
